Emergency information form
One page for 911, the ER and new caregivers: conditions, medicines, devices, baseline and the care team. Print it, or keep it on your phone.
Private by design: what you type stays in this browser on this device. Nothing is sent to us. Clearing your browser data erases it, so download a copy now and then.

Emergency teams see your child for the first time. This one page tells them what is normal for your child, what they take and use, and who to call. It is modeled on the Emergency Information Form recommended by the American Academy of Pediatrics and the American College of Emergency Physicians. Ask your doctor to look it over.
Where to keep it
- In the go-bag, on the fridge, and as a photo on your phone.
- With the school nurse and every caregiver.
- Update it after each hospital stay.
